TRANSCRIPT ORDER FORM:
IMPORTANT NOTE: Emailed, faxed, and personally picked up transcripts are UNOFFICIAL. Only transcripts mailed directly or electronically submitted from SJS to a program or university are considered OFFICIAL. Current school year transcripts are released with Upper School historical data up to the most recent end of semester. Indicate below how you would like your transcript processed.
If you are an NCAA or NAIA athlete, DO NOT submit this request. Mrs. Leakey will automatically upload transcripts for student athletes who have current NCAA or NAIA accounts once transcripts are officially released. If you are a recent graduate, DO NOT SUBMIT this request as the College Counseling office will submit your official transcript to your college as soon as it is released. Mrs. Leakey has no part in that process.
Please make your transcript request at least two business days before scheduled holidays and break periods as any requests received during that time will undergo extremely delayed processing.

TRANSCRIPT PROCESSING INFORMATION:
Normal processing time is two to three business days from the date the transcript request is received (note, if your transcript request is sent over a weekend, it will not be processed until the next business day, usually Monday, except for official school holidays). Peak periods such as grade reporting periods, the beginning/end of semesters, and official holiday breaks (Thanksgiving Break, Winter Break, Spring Break, Summer Break) when the offices are closed WILL result in extended processing times of approximately two to four weeks. This processing time excludes delivery.
We strongly recommend that any transcripts needed during break periods be requested at least two to three business days prior to the break period. Please submit only one request per person. Submitting multiple requests will not speed up transcript processing, as requests are handled from oldest to newest.

NOTE: This transcript request form applies to Upper School and alumni/former students only. If you need Middle School records, please contact Kelly Blocker at kblocker@sjs.org. If you need Lower School records, please contact Melissa Narciza Garcia at mnarcizagarcia@sjs.org.
TRANSRIPT HOLDS:
No transcript will be released if there are outstanding obligations to the School. In the event that you have a hold on your transcript, you will be contacted at the email or phone number you provided above.
PRIVACY ACT OF 1974:
In keeping with the Privacy Act of 1974, transcripts will only be released to the student or the student's parent/guardian. Persons picking up the transcript may be asked to provide picture ID.
